The Student Health Center is located on the second floor of the Granito Center and staffed by nurse practitioners. Scheduled appointments are preferred when possible. There are no charges to students for routine services. Minimal fees will be charged for immunizations and some diagnostic or screening tests. Hours of operation are posted and subject to change. Immunizations: By mandate of two New York State laws, all students must provide proof of immunizations for measles, mumps, and rubella and they must submit a form to the Health Center acknowledging having had an immunization for meningitis or indicating that they are aware of the risks of not having such an immunization. New students can locate documentation of immunization at their former high school, college, and/or health care provider. This documentation must be submitted to the Health Center within 30 days of enrollment. Failure to do so will, by New York State mandate, result in the revoking of the privilege of attending classes. The meningitis vaccine is required for all athletes and resident students. Confidentiality: All services provided by the Student Health Center are confidential. Information will not be released without the written permission of the student, except in an emergency situation where the safety of the student is at risk.
The MMR Immunization Documentation and Meningitis Response Form must be completed and returned to the Health Center to complete your registration requirements.
